An intubating bougie (or gum elastic bougie) is a flexible device used as an adjunct during difficult endotracheal intubation. It helps guide the endotracheal tube (ETT) into the trachea when direct visualization of the vocal cords is challenging.
Key Features:
Flexible & Rigid Enough – Can be manipulated but maintains shape.
Angled Tip (Coude Tip) – Facilitates navigation into the trachea.
Surface Markings – Helps gauge insertion depth.
How It’s Used in Intubation:
1. Insertion First – The bougie is inserted through the vocal cords.
2. Confirmation – Feel for tracheal "clicks" or resistance at the carina.
3. ET Tube Advancement – The ETT is slid over the bougie into the trachea.
4. Bougie Removal – The bougie is carefully withdrawn, leaving the ETT in place.
