When to deliver: - Mild preeclampsia = 37 weeks - Severe preeclampsia = 34 weeks (or asap in certain conditions like eclampsia, hellp, pul edema, fetal distress, abruption, Dic, worsening renal function) - GDM well controlled on diet: after 39 weeks - GDM on OHA: 38 weeks - Overt DM well controlled: 38 weeks - overt DM poorly controlled: 37 weeks - IHCP: 37-38 weeks - AFLP: stat - HeLLP: Stat - eclampsia: Stat - Abruption: Stat - Placenta previa: Conservative if patient stable, try to take till 37 weeks - Placenta Accrete syndromes: 34-37 weeks plan cesarean + hysterectomy - Rh neg: 40 weeks - Rh isoimmunized but ni fetal anemia (MCA doppler normal): 37 weeks - Rh isoimnunized with fetal anemia: 34 weeks - IUGR with normal umb artery doppler/ reduced diastolic flow: 37 weeks - IUGR with absent or reversal of diastolic floe on umb artery doppler: if > 34 weeks: stat. If< 34 weeks: steroids then deliver. - DCDA twins: 38 weeks - MCDA twins: 34- 37 weeks - MCMA twins: Elective Lscs at 34 weeks - Breech/ Transverse lie: Try ECV if indicated between 36-37 weeks.
