🔴The Dark Room Prone Provocative Test (DRPPT) is a clinical test used primarily to detect angle-closure glaucoma in patients with narrow angles. It helps assess the risk of angle closure under conditions that mimic physiological pupillary dilation in dim lighting.
*Procedure:*
1. The patient is placed in a completely dark room.
2. They lie prone (face down) for 30-60 minutes.
3. This position promotes choroidal congestion, which can push the iris-lens diaphragm forward, narrowing the anterior chamber angle.
4. After the stipulated time, intraocular pressure (IOP) is measured and compared with the baseline.
*Interpretation:*
•Significant IOP increase (≥8 mmHg) suggests a predisposition to angle closure.
•No significant change indicates a lower risk.
This test is particularly useful in subclinical or intermittent angle closure cases where gonioscopy alone may not be conclusive. However, it has been largely replaced by more advanced imaging techniques like AS-OCT and UBM for better anatomical assessment.
