🔺 PLIHA ROGAS( SPLEEN DISORDERS)
🔺DIFFERENTIAL DIAGNOSIS OF SPLENIC DISEASES
📌Infectious mononucleosis Fever, sore throat, lymphadenopathy, splenomegaly
📌Malaria:Cyclical fever, anemia, splenomegaly, parasitemia
📌Typhoid fever:Step-ladder fever, abdominal symptoms, mild splenomegaly
📌Kala-azar (Visceral leishmaniasis):Massive splenomegaly, pancytopenia, hyperpigmentation
📌Portal Hypertension Splenomegaly with ascites and esophageal varices
📌Hemolytic anemia:Jaundice, pallor, reticulocytosis, splenomegaly
📌Leukemia/Lymphoma Systemic symptoms (fever, weight loss, night sweats), lymphadenopathy, massive spleen
📌Splenic Abscess:Fever, LUQ pain, tenderness, leukocytosis
📌Splenic Infarct: Sudden LUQ pain, tenderness, fever, elevated LDH
📌Gaucher’s Disease Hepatosplenomegaly, anemia, bone pain in children/young adults
📌Sarcoidosis or Amyloidosis Systemic features, splenomegaly on imaging, tissue biopsy needed
🔺 SAMPRAPTI (PATHOGENESIS)
-The pathogenesis involves Vata dosha and Rakta dhatu. Vitiated Vata enters and vitiates Pliha leading to:
-Dosha: Predominantly Vata, secondarily Rakta and Pitta
-Dushya: Rakta, Rasa, Mamsa, Meda
-Srotas: Raktavaha Srotas
-Srotodushti: Sanga (obstruction), Atipravritti (excess flow)
-Udbhava Sthana: Amashaya
-Adhisthana: Pliha
-Vyadhi Swabhava: Chirakari (chronic), Kashtasadhya
🔺UPASHAYA (PALLIATIVE / RELIEVING FACTORS)
-Tikta (bitter), Kashaya (astringent), Laghu (light) diet
-Mild swedana (sudation)
-Deepana and Pachana drugs (appetizers and digestives)
-Proper elimination therapy (Mridu Virechana or Basti)
-Use of medicated ghrita with Vata-pacifying herbs
🔺 ANUPASHAYA (AGGRAVATING FACTORS)
-Intake of Guru, Snigdha, and Abhishyandi ahara (heavy, unctuous, clogging food)
-Diwaswapna (day sleep)
-Suppression of natural urges
-Physical exertion or trauma
-Alcohol consumption

-The general approach includes -Vata-Rakta pacification

-Srotoshodhana: Trivrit, Haritaki
-Virechana: Using Trivrit Lehya, Draksha, Aragvadha
-Basti Chikitsa: For Vata predominance – Eranda Taila, Dashamoola Niruha Basti
-Rakta Shodhana: Use of Manjistha, Guduchi, Daruharidra
-Rasayana: Use of Amalaki, Guduchi, Bhumyamalaki
