✅ Key Message:
Nifuroxazide is not routinely recommended in the treatment of acute gastroenteritis in children.
✅ What You Should Know:
• Acute gastroenteritis is usually viral in origin in children — treatment is mainly supportive with rehydration.
• Nifuroxazide, a nitrofuran derivative, is a locally acting intestinal antiseptic with limited systemic absorption.
• Evidence is insufficient to recommend its routine use in children, especially in high-income and guideline-driven settings.
✅ When Might It Be Used?
• In some countries, it is used empirically for presumed bacterial diarrhea when:
• There is no access to rapid stool testing.
• Symptoms suggest possible bacterial etiology (e.g., fever, dysentery, travel-related diarrhea).
• A short course (≤3 days) may be used cautiously in older children.
✅ Clinical Pearls:
• Oral rehydration therapy (ORT) remains the cornerstone of treatment.
• Overuse of antimicrobials like Nifuroxazide may promote antimicrobial resistance.
• Do not use Nifuroxazide in:
• Infants <2 months.
• Suspected bloody diarrhea, systemic infection, or in place of proper antibiotics.
✅ Guidelines Reference:
• ESPGHAN/ESPID 2014 Guidelines: Antimicrobials generally not recommended in routine acute diarrhea.
• WHO 2023 Guidelines: Emphasize ORT, zinc, and nutritional support over antimicrobials.
