ALT OR AST mild elevated 😱😱😱

Controversies in Pediatric Gastroenterology
Should We Screen Asymptomatic Children for Liver Enzymes?
⸻
Clinical Dilemma:
During a routine check-up, a healthy, asymptomatic child has liver enzymes ordered “just to be safe.” ALT returns mildly elevated. What’s the next step?
⸻
What’s the Controversy?
• Routine liver enzyme screening in asymptomatic children is becoming increasingly common, but not universally recommended.
• Mild, transient transaminitis is frequent and often benign in children (e.g., post-viral or non-alcoholic fatty liver).
• Over-screening leads to unnecessary anxiety, investigations, and referrals without improving outcomes.
⸻
Current Best Practice:
• Do not routinely screen liver enzymes in healthy, growing, asymptomatic children unless there’s:
• Family history of liver disease
• Obesity or metabolic syndrome
• Signs like hepatomegaly, jaundice, or growth failure
⸻
Bonus Insight:
ALT alone is not a diagnosis — interpret in clinical context. Routine testing in well children may do more harm than good
