This recommendation follows a significant incident in February 2021, where the main cargo door of a DHL-operated Boeing 757 opened mid-flight, leading to serious safety concerns. On February 13, 2021, a Boeing 757 converted freighter registered G-DHKZ departed from DHL’s European hub at Leipzig-Halle Airport (LEI) in Germany. Shortly after takeoff, the aircraft experienced a pressurization failure. The crew later discovered that the main deck cargo door had not latched correctly, resulting in its uncommanded opening at approximately 5,000 feet. The flight crew reported difficulties with the autopilot and chose to manually fly the aircraft back to Leipzig, landing safely about 20 minutes after takeoff. This incident is not isolated; a similar occurrence happened in 2014 involving a Boeing 757 converted freighter operated by Yakutia Airlines in Russia, where malfunctions led to the cargo door opening mid-flight. The FAA's recent Special Airworthiness Information Bulletin (SAIB) emphasizes the importance of inspecting the cargo door systems of 757 conversions performed by Precision Conversions LLC. Operators are instructed to check for contaminants, smudges, and other debris on viewport mirrors and windows. Additionally, they should examine latches and lock pins for any visible paint defects, and follow the appropriate cleaning and painting protocols if necessary. The FAA also highlights the need for crews to implement specific operational procedures in cold weather.
